Opportunity Description
Position Summary
We are seeking a compassionate and experienced Admissions Registered Nurse (RN) to join our hospice team. The Admissions RN is responsible for evaluating patients for hospice eligibility, completing admission assessments, coordinating the start of care, and educating patients and families about hospice services. This role requires excellent clinical judgment, strong communication skills, and the ability to work collaboratively with interdisciplinary team members to ensure a smooth transition into hospice care.
This position will oversee the Bergen/Essex/Union county area.
The hours are Monday-Friday 11am-7pm
Responsibilities
+ Conduct comprehensive hospice admission assessments in patients' homes, facilities, or hospitals.
+ Determine hospice eligibility in collaboration with the hospice physician and interdisciplinary team.
