Summary The incumbent performs a variety of technical support duties that facilitate the work of physicians, physician assistants, nurses and other members of the medical team who provide direct patient care. This position is located at the Rainbow City Clinic, Gadsden, AL. Responsibilities Works collaboratively in an interdisciplinary coordinated care delivery model in a Specialty Clinic or with Patient Aligned Care Team (PACT). Coordinates with the patient care team to review the clinic utilization by using various reports (e. g., Clinic Utilization Statistical Summary). Ensure that the clinic setup is closely monitored to effectively support the needs of the clinic and make any necessary adjustments. Develop and maintain effective and efficient communication with the patient interdisciplinary team VA medical centers and other agencies.
