We have an exciting opportunity for a Band 8a Advanced Clinical Practitioner (ACP) committed to delivering high quality care to join our Frailty (Care of the Elderly) team. The successful candidate will provide comprehensive geriatric assessment (CGA) for frail older adults and be part of a team developing the Frailty provision, which includes integrated working between the Emergency Department, wards and primary care & community services.

Main duties of the job

As an autonomous practitioner, you will ensure your practice is underpinned by the four pillars of advanced clinical practice.
