Description: MALE SUPPORT WORKER ONLY Personal care 1 hour Showering Level of assistance: Supervision- leaning towards physical assistance Equipment in use: Shower chair Mobility /Transfers: Walter uses his four-wheeled walker and walking stick Setting up area (clothes, towel etc): towel is on the rail in the bathroom. Undressing/ dressing: Only supervising, help when he needs. Assistance in the shower: Assisting Walter only if he needed Getting dry: No assisting just supervision. Special requirements: Please ask discuss with family the level of assisting. The client's daughter suggested that client is able to do it independently however due to his fragile and falls it is best to assist him as much as we can.
