(Assistant) Nurse Clinician (Dementia Care)

The (Assistant) Nurse Clinician (Dementia Care) supports the delivery of person-centred, evidence-informed nursing care for older adults living with dementia across St Luke's ElderCare's centre-based, community-based, home-based, and residential care settings. The role provides clinical assessment, care planning, caregiver education, and team support to enhance the safety, dignity, functional ability, and quality of life of seniors with dementia, in alignment with St Luke's ElderCare's philosophy of care and whole-person approach spanning physical, psycho-social, cognitive, and spiritual domains.
