Behavioral Health Coordinator

Position Summary This individual will provide behavioral health support to residents of long‐term care facilities under the supervision of a licensed professional. They may receive training in a broad range of assessments and interventions that support treatment specified in individual care plans. Regional travel will be required. Essential Duties And Responsibilities Conduct assessments that include the behavioral and social needs of the patients. Meet with families and facility staff to coordinate care as recommended by licensed professionals. Assess the patient's knowledge of their clinical condition and provide education and self‐management support. Provide outreach to clients who are having problems with treatment adherence. Assist in the development of the individual goals and behavioral health treatment plan. Coordinate with other agencies and community providers. Maintain timely documentation of work.
