Summary
You’ll support a high-volume care coordination team by making outbound calls to patients regarding pharmacy refills, annual wellness visits, ER outreach, and new authorization needs. Additional project work will be assigned by leadership as priorities shift.

Key Responsibilities

+ Make outbound calls to patients regarding pharmacy refills, wellness visits, and ER follow‑up

+ Support new authorizations and insurance/benefit verifications

+ Perform accurate data entry and documentation

+ Complete Excel-based tasks (hide, filter, copy, paste data)

+ Assist with special projects delegated by leadership
