Care Coordination RN — Home-Based & Telehealth Impact

Mosaic Community Health in Somerset West is seeking a Registered Nurse (RN) to provide patient-centered care and coordinate care for complex patients. This role requires a minimum of two years of clinical experience in acute or ambulatory settings and a Bachelor of Science in Nursing (BSN) is preferred.
