Care Coordinator (Case Management Coordinator)

**Position:** Care Coordinator

**Hourly Rate:** $28.71 / hour

**Employment Type:** Full-Time

**Schedule:** Monday–Friday, 8:00 AM – 5:00 PM (with flexibility required to meet member needs)

**Location:** Field-Based Telework (Regional)

+ **Travel Requirements:** Up to 50% or more regional travel to member homes and other requested locations. Onsite meetings at the corporate office are required 1–2 times per year.
+ **Residency Requirement:** Applicants must reside within the designated hiring region/counties.
