Job Description

The candidate will provide service coordination management, administrative support, and operations improvement and analysis for our Community Rehabilitation services and Senior Gym programmes. You will work alongside passionate and experienced co-workers, and guidance will be provided. If you are customer-oriented and have an eye for detail, you will enjoy this role.


Key Responsibilities

a) Service Coordination Management

  • Manage the onboarding process for new clients, assist in appointment scheduling and re-scheduling, subsidies application and address client enquiries for Community Rehabilitation services and Senior Gym programmes

  • Work closely with internal and external stakeholders (where applicable) to ensure the smooth delivery of services.
  • Active Rehabilitation (AR) services by Therapists.

  • Circuit Training, Circuit Training Focus and Functional Strength Training by Exercise T...
    • Full Time Home Health and Personal Care Aides; and Nursing Assistants, Orderlies, and Psychiatric Aides
