Duties: Support clinical team in advising clients on financial scheme applications, ensure timely update of renewal of means testing and financial assistance Support the clinical team in administrative processes in the delivery of services to clients and caregivers Manage onboarding process for new clients, assist in initial assessment appointment scheduling/ rescheduling, subsidies application, and address client inquiries for rehabilitation Assist in initial assessment of client/caregiver needs with basic case management under the direction of a case manager/ social worker Ensure client and caregiver's ability to cope by helping to streamline and coordinate the range of community services available Assist client/caregiver with appropriate information and referral assistance Conduct joint home visits with a case manager/ social worker or the multidisciplinary team where necessary Support events or projects in relation to client and caregiver support as planned Participate in case disc...
