Responsibilities Conduct assessment and treatment of patients along the continuum of care. Develop and implement discharge planning within an interdisciplinary healthcare team. Identify and remove barriers that impede access to post‐acute care. Address complex clinical care needs, including substance abuse, by collaborating with providers. Lead family meetings, provide bereavement support and grief counseling. Advocate with community agencies and within the acute care interdisciplinary team.
