This position follows a hybrid schedule with three in-office days per week.
**Primary Responsibilities:**
+ Assist in the care of patients through telephonic outreach to identify care needs and barriers to care, connect patients to community resources, and schedule hospital follow up appointments and AWV's as needed
+ Obtain pertinent medical records for discharged patients to support provider decision making during HFU appointments
+ Complete medication review of new and changed medications for patients discharged from a facility
