We are seeking a full-time bilingual (English/Spanish) Care Manager to join our innovative High Fidelity Wraparound (HFW) team. HFW is an evidence-based process that supports youth with severe emotional and behavioral needs. HFW provides team-based planning, that is comprehensive, well-coordinated, holistic and family/youth-driven, with the intent to meet the complex needs of youth with serious mental health needs.
