Care Manager - RN

Qualifications

• Graduate of an accredited School of Nursing.
• Currently licensed as a Registered Nurse in the State in which you are applying (or another NLC state).
• 2+ years of previous experience working in care management and/or with CKD/ESRD patients is preferred.
• Ability to take calls remotely on some nights and weekends
• Must have Home Health and/or Hospice experience.
