Care Navigator/Case Manager

Reports to:

• TCM Coordinator & Clinical Supervisor Role- What you do This position is responsible for the case management of mental health/substance use 25 individuals; for creating and insuring implementation of individualized care plans to address significant areas.


• Link individuals and families to needed resources, coordinate services, monitor progress and services, and facilitating meetings and monthly visits.
• Ensures that each individual receives services appropriate to the youth’s individual care plan.
• Enters case notes in a timely manner, documenting all interactions with youth, family, involved agencies, schools, etc., concisely summarizing those interactions.
• Collaborates with the therapist and other service providers to develop a individualized care plan with the involvement of the youth’s family and any involved agencies.
