Care Partner - Aged Care

As a Care Partner, you’ll guide customers through their care journey with compassion. Working closely with families, clinical teams, and external providers, you’ll develop holistic health plans, coordinate services, and ensure customers receive safe, person-centred care that enables them to age well at home or in a place of their choosing.

This is a hybrid, full-time role, with office location in Bankstown and supporting customers across Fairfield and Bankstown LGAs.
