Responsibilities
+ Provide case management and care coordination services to clients under supervision, including connecting individuals to resources, benefits, housing, healthcare and collaborating with external providers and agencies to ensure continuity of care.
+ Review, organize, and update community resource and referral dashboards/folders to ensure accurate and current information.
