Case Manager, Permanent Supportive Housing Program (WRCC WINDHAM)

Direct Case Management Functions:

· Establish rapport and working relationships with assigned clients, and work within a team approach to assist with all clients within the PSH program

· Provide initial and ongoing assessment of client needs and strengths

· Develop and write comprehensive client service plans with client input and conduct regular reassessments of needs

· Assist with understanding Federal guidelines/expectations of rent subsidies, tenant obligations and landlord relationships

· Assist with crisis prevention/intervention

· Perform home visits to client’s homes as needed and provide transportation for clients to secure benefits, find employment, obtain medical care and meet basic needs



Maintains Client Contact and Client Centered Services:

· Liaison with landlords to establish appropriate housing options for clients

· Assist in linking client with community support systems and agencies
