Chronic Care Management Coordinator

LaSante Health Center is seeking a Chronic Care Management Coordinator to join our team. The ideal candidate will serve as the primary point of contact for patients and families, guiding them through their care journey and connecting them to internal and external resources as needed. Key Responsibilities
+ Act as the main point of contact for patients and their families regarding all aspects of care received at LaSante.
+ Monitor and manage a designated panel of patients, particularly those with chronic health conditions.
