Clinical Care Manager

The Clinical Care Manager is an integral member of the multidisciplinary PCMH team, supporting high-risk and high-cost patient populations through care coordination, patient engagement, chronic disease management, and transitional care services.

The Clinical Care Manager collaborates closely with primary care providers, nurses, social workers, medical assistants, administrative staff, specialists, community partners, and other stakeholders to improve patient outcomes, enhance the patient experience, and reduce avoidable healthcare utilization and costs.
