Clinical Case Coordinator - Albany NY

Job Summary: The Clinical Case Coordinator works with a wide range of patients with work-related illness and injury. Working within the scope of their nursing license (RN or LPN), the Coordinator ensures that quality care and services are provided and documented in a consistent manner and in alignment with each patient's individualized care plan and OECC policies. The Coordinator arranges and expedites the diagnostic and treatment process and facilitates care across all clinical cases by managing, coordinating, and monitoring resources to achieve optimal clinical outcomes and appropriate utilization of services. As a contributing member of a multidisciplinary team, the Coordinator helps ensure safe and appropriate clinical pathways.
