Overview


Physical Therapist / Clinical Coordinator
Seeking an experienced Physical Therapist interested in care coordination, neurorehabilitation, and team leadership.
In this dual-role position, you will provide physical therapy services while serving as a key team lead and central point of contact for coordinating patient care. You'll help facilitate individualized rehabilitation plans, collaborate with the rehab team, and support successful community reintegration for individuals recovering from neurological injuries and complex conditions.
This role would be the perfect fit for:

+ Current Physical Therapist licensure required* in the State of Texas

+ Clinicians who enjoy balancing direct patient care with care coordination responsibilities
