Opportunity Description
Altrincham Healthcare Alliance Primary Care Network (PCN) is looking to recruit a Independent Prescribing Clinical Pharmacist to join a PCN of five GP Practices covering 48,000 patients in South Trafford. Working in practice 30 hours per week, the ideal candidate will be a forward-thinking, enthusiasticindividual with initiative who is keen to work within a team of like-mindedpeople and deliver excellent pharmacy-led services. You will become part of an evolving team of 8 Clinical Pharmacists withclinical support from the Lead GP.
Working across the five GP practices within Altrincham HealthcareAlliance PCN, you will work closely with GPs, nurse practitioners, social prescribersand existing pharmacists as part of a multi-disciplinary team while alsobuilding strong effective working relationships with the local community,hospitals, the PCN community pharmacy lead and wider pharmacy teams to improvethe care and services to our patients.
