Clinical Pharmacist (Formulary Clinical Pharmacist)

Summary The formulary clinical pharmacist is a licensed professional with 1 year of experience equivalent to the next lower grade level or a completion of an ACPE-accredited Pharm.D. program, and the knowledge and proficiency in pharmacy practice, pharmaceutics, pharmacotherapeutics, facilitating efforts in the review of clinical documentation and the appropriate formulary to determine appropriate medication therapy, and providing prior-authorization services for formulary medications.
