Clinical Pharmacist

Summary The St. Louis VA Healthcare System is seeking a Clinical Pharmacist that facilitates resolutions of Veteran concerns. They will be responsible for interpretation of provider medical orders, prescription order/entry/completion, patient compliance monitoring, patient education, provider drug information consultations, and patient medication therapy monitoring. Responsibilities Handles routine medication-related activities. Makes recommendations to clinical staff regarding medication therapy. Documents recommendations and interventions. Accurately fills or checks medication dispensing units, prescriptions, and aseptically prepared intravenous admixtures and other aseptically prepared pharmaceuticals.
