Detailed Overview Supporting the Vision, Values, Purpose, and Commitments of Fraser Health including service delivery that is centered around patients/clients/residents and families, the Clinician, Child and Youth Transition Team provides comprehensive assessment, treatment and referral coordination for children and youth ages 6-18 years of age and their families in acute crisis, educates and consults with hospital and community personnel, networks with community agencies, participates in research and assists in program and resource development.

Responsibilities

• Conducts comprehensive assessments to identify client and family's social, emotional and cultural needs, in order to formulate appropriate treatment recommendations.
