The Community Based Service Associate (CBSA) will be the first point of contact for those individuals seeking services with CKP. The CBSA will have strong knowledge of all CKP programs and services and how individuals can efficiently access those services, including the workflows for referral, intake, eligibility, screening, and evaluation. The CBSA will serve as a resource to all clients, staff, and community members in eliminating barriers for patients related to social determinants of health and the acquisition of services. The CBSA will work from the CBS Building on Ferry Street and assume responsibility for answering all incoming calls, taking messages, directing calls to appropriate staff throughout the agency, and documenting incoming referrals for follow-up later.
