This is an exciting new role suitable for a qualified HealthCare Assistant who will provide planned and unplanned care for practice patients in their own homes.

The role holder will be an experienced Health Care Assistant currently working in Primary or Community Care. They will be expected to work autonomously within the nursing team to deliver excellent patient care, in the community.

• Work in partnership with individuals, families, and carers to deliver patient-centred care
• To monitor care in line with practice protocols
• Undertake face to face consultations at the patients home to deliver care.
