Community Healthcare Support Worker - Portsmouth area

The role is based at Portsmouth and involves working within the Integrated Care Team to deliver community‑based services to the GP Practice population. The responsibilities include nursing tasks carried out within the parameters set out by registered practitioners, home visiting, lone working without direct supervision, supporting admission avoidance, early supported discharge, promotion of independence and effective management of patients with long‑term conditions, older adults and those with mental health problems.
Full UK driving licence and access to a car are essential. The Trust does not offer Skilled Worker visa sponsorship; applicants must have the right to work in the UK.
