Community Mental Health Nurse

The Community Mental Health Nurse will work as part of one of the four Neighbourhood Mental Health Team in Tower Hamlets, providing high-quality, person-centred care to adults experiencing complex mental health needs.

The post holder will be essential to delivering care that focuses on response to crisis presentations, delivery of early intervention and proactive management that leads to reduction in admissions for high-intensity service users, reduction in ED cubicle utilisation, and improved use of inpatient capacity.

They should foster Community and Connection and should be compassionate and show empathy.

The post holder should have a genuine desire to work in a multi-disciplinary and multi-agency team.
