Responsibilities

• Assist in managing the clinical care of patients with mental ill health as delegated by Senior Practitioners (CPN) for adults over age 65 or any age with memory concerns.
• Conduct assessment; care planning, implementation and evaluation of the care plan in partnership with client, family and carers.
• Monitor medication, liaise with psychiatrists, primary care and other social care services, and refer to other services as appropriate.
• Serve as an active member of the multidisciplinary team.
