An opportunity for a Band 6 Community Mental Health Practitioner (Nurse/OT/Social Worker) to join a dynamic team in Cannock.
About the role
- Take responsibility for the management of a complex caseload, including specialist assessment, care planning with service user and carer, and evaluation of individual treatment plans.
- Participate in setting quality standards, auditing, monitoring and reviewing practice in line with current clinical guidance and policy.
- Administer medication, complying with relevant Trust procedures and NMC guidance.
- Assess the patient’s individual care needs, develop, implement, and evaluate programmes of care.
