This post will involve working in outpatient clinics providing both rapid access, admission avoidance clinics for the frail population of Hillingdon and general elderly medicine clinics.

Main duties of the job

The current set up of this post is for 2 full days of clinics and SPA worked flexibly so it will suit someone looking for less than full time or flexible working. Our Rapid Access Clinics for the Elderly (RACE) run up to 5 times a week providing rapid assessments of community dwelling frail patients referred by their GP and also follow up for patients seen by our service on the wards or in our Acute Frailty Unit supporting ED.

General clinics serve a wide range of presentations in over 65s where a holistic approach to care is required. We have recently expanded our acute frailty provision and provide 7 day a week frailty SDEC services. Joi...
