To support the Mission of Helio Health, Inc., theCTI Licensed Practical Nursewill provide comprehensive care management and coordination of services to Critical Time Intervention (CTI) program enrollees. The CTI model is a time-limited form of case management that focuses on helping people with severe mental illness (SMI) and a history of homelessness during a “critical time” transitional period in their lives.
CTI Licensed Practical Nurse:

CTI is designed specifically to enhance the continuity of care during transition from hospital to community care. Helio Health is on a mission to treat and promote recovery from the effects of substance use, mental health disorders, and other behavioral healthcare issues.
