Dementia Post-Diagnosis Support Navigator

NHS Scotland is looking for a Dementia Link Worker to join the Older Adult Community Mental Health Team based at Queen Margaret Hospital, Dunfermline. The successful candidate will manage their own caseload and provide Post Diagnostic Support to individuals newly diagnosed with dementia.

Responsibilities include offering advice to patients and their carers, and collaborating with the healthcare team. Applicants should have experience with older adults and demonstrate strong communication skills. The position requires a commitment to equality and diversity.
