About the Role

An opportunity has arisen to join Secure Services Guild Lodge as a Forensic Discharge Pathway Facilitator (Inpatient Social Worker).

Key Responsibilities

• Play an integral role as part of the Multi-Disciplinary Team.
• Work in close partnership with a range of agencies and organisations internal and external to LSCFT to offer social care input.
• Take a lead role from pre‑admission by completing a holistic social work assessment, identifying any social work needs, undertaking family work, and supporting discharge planning.
• Identify early barriers to discharge and take action to prevent unnecessary delay.
• Support secure mental health inpatient care for Lancashire and South Cumbria at Guild Lodge.
• Assist with the Men’s Care Pathway, Women’s Care Pathway and Forensic Brain Injury Service bed management.

Qualifications

• Qualified Social Worker.
