Discharge Support Officer

An exciting opportunity has arisen to join an dynamic forward thinking Discharge and Transfer Team. We are looking for enthusiastic and motivated team players with excellent communication, problem solving and organisational skills. If you want to grow your discharge skills and knowledge in a supportive team then this is an ideal opportunity. You will work alongside the Discharge and Transfer leads and ward teams to provide support and advice on discharge. This role includes completing referrals to other services, working to tight deadlines and communicating with patients, families and the wider MDT. Being a point of contact with both the ward team, patients and families is vital to help reduce the length of stay for patients.
