Opportunity Description
Great group in Central Western Oregon about 1hr 20mins from Portland has Family NP/PA and Internal Medicine NP/PA support openings. 
- M-F, forty (40) work hours per week, including a minimum of twenty-four (24) hours availability for patient contact per week.
- Providing same-day or next-day access for patients with urgent, low acuity care needs;
- Providing interim co-management of chronic illnesses (titrating medications, interval routine follow-up care)
- Conducting, as needed, CMS Annual Wellness Visits; and,
- Meeting on a regular basis with internal medicine providers to review patient charts, collaborate on patient cases, and discuss workflow and expectations.
- The role will support up to, but not exceed, four (4) primary care providers.
- Neither management of a primary care panel nor call will be assigned responsibilities.
