Opportunity Description
We are seeking an experienced NursePractitioner or Physician Assistant for a Locums assignment to convert toPerm (with in 3-6 months) at a primary care practice serving geriatric patients ina value-based care model. This is a hybrid role with blend ofcoverage in outpatient clinics as well as home-based patient care.
Compensation: $75 - $95 perhour
Position Details:
- Schedule: Full Time Hours Guaranteed
- Monday - Friday - 5 Days Per Week
- Patient Load: LOW DAILY CENSUS
- Home Care: 3-5 patients per day
- Clinic: Small census of 7-10 patients per day
- Setting: Hybrid (Outpatient clinic + home-based care), value-based primary care for elderly patients
- Locations: Bronx, Brooklyn and Ridgewood, NY
