HEALTH CARE WORKER - COMMUNITY SUPPORT

Fixed Term Part Time – 12 - 76 hours per fortnight

We are looking for a Health Care Worker to join our supportive Community Aged Care Services team at Portland District Health.  As a Healthcare Worker – Community Support, you will assist consumers with:

• Light, general housekeeping such as sweeping, vacuuming, mopping, dusting, wiping, bed making, laundry, and limited shopping.
• Provide in home respite or participate in social activities like shopping, outings and walks that assist them to stay active and connected with their community: including one on one and/or group activities.

• Encouraging consumers to maintain and enhance their abilities and strengths.
• Encouraging consumers to actively participate where possible to maintain their independence.

• Support the individual’s choice to remain at home and participate in their community.
  Part time Home Health and Personal Care Aides; and Nursing Assistants, Orderlies, and Psychiatric Aides
