**Job Summary and Responsibilities**
As our Home Health Aide CNA, you will provide essential personalized support and companionship to clients in their homes, enhancing their overall quality of life and fostering independence according to individualized care plans.
Every day you will assist clients with activities of daily living (ADLs) such as personal care, meal preparation, medication reminders, light housekeeping, and transportation to appointments. You will meticulously document services provided and proactively communicate any changes in the client's condition to the appropriate supervisor.
To be successful in this role, you will possess a compassionate and patient demeanor, excellent communication and interpersonal skills, and a strong commitment to client dignity and well-being.
