Activa Home Health, An AccordCare Company is seeking experienced and compassionate Field Licensed Practical Nurses, LPNs to provide per diem home health visits throughout Palm Beach County and the surrounding areas.

We are a licensed, Medicare-certified home health agency committed to delivering exceptional patient care while supporting our nurses with flexibility, teamwork, and strong clinical leadership.

As a Field LPN, you will provide skilled, patient-centered care in the home setting, following individualized Plans of Care under the supervision of a Registered Nurse. Your work will help patients remain safe, comfortable, and independent in their homes.
