Make a meaningful impact every day as a CenterWell Home Health Nurse. You will provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you will develop and manage care plans that support recovery and help patients get back to the life they love. You will report to the Clinical Manager.
As a Home Health Registered Nurse, you will:
+ Provide admission, case management and follow-up skilled nursing visits for home health patients, as directed by the Physician and Clinical Manager
+ Coordinate appropriate care, encompassing necessary healthcare personnel (such as Physical Therapists, Occupational Therapists, Licensed Practical Nurses, Home Health Aides and external providers)
+ Report patient care, condition and progress to patient's physician and the Clinical Manager
+ Prepare appropriate medical docu...
