Home Visitor (Grant) – Community Outreach – St. Vincent Medical Center

**Primary Function/General Purpose of Position**

Provides home visitation services to expectant moms and families with young children that cultivate and strengthen nurturing parent-child relationships, promote healthy childhood growth and development, and enhance family functioning by reducing risk and building protective factors.

**Essential Job Functions**

+ ​Provides culturally sensitive home visitation services for families that promote positive parent-child interaction, child development skills, and health and safety practices using a curriculum and other educational materials in accordance with best practice model standards. Provides information, referrals, and linkages to available community resources. Links families to a medical provider to assure optimal health and development (i.e., timely immunizations, well-child care, etc.).
