Our Hospice Liaison RN’s :

+ Reviewing patient referrals, discuss hospice care options with patients and family members, and plan with both the patient and the facility to prepare for hospice care.

+ Develops a positive working relationship with doctors, nursing facilities, and hospitals who refer clients to hospice.
