Our Lady of Consolation is located on the beautiful Great South Bay and is comprised of a 345-bed nursing and rehabilitative care center. The Hospital Assessment Liaison serves as the primary connection between hospitals, patients, families, and the nursing home admissions team. This role is responsible for coordinating referrals, reviewing clinical information, completing patient assessments, and facilitating smooth admissions and transitions into the skilled nursing facility.
