What you’ll be doing

• Experience in strengths‑based practice and working in a fast‑paced, pressured environment, particularly within the Discharge to Assess (D2A) pathway.
• Strong assessment and care planning skills, with an understanding of statutory duties, relevant legal frameworks, and funding streams to ensure safe, timely, and cost‑effective care planning.
• Ability to collaborate effectively within a multi‑disciplinary team, fostering positive relationships with colleagues and service users.
• Demonstrated commitment to delivering person‑centred outcomes and making a real difference in the hospital setting.
