Clinical Role:

* Family Medicine physician
* Internal Medicine physician
* Medicare-focused patient population (65+)
* Hybrid schedule
* Four onsite days per week (8:00 AM-5:00 PM)
* One remote day per week
* Monday-Friday schedule
* Part-time option available
* Light call schedule
* Approximately two evenings per month
* Approximately two weekends per year
* Average patient volume: 15-18 patients per day
* Comprehensive, team-based primary care model
* Focus on chronic disease management, preventive care, and social support
