• Clean Malpractice
• Michigen (MI) License
• Does the provider require visa support - J1 or H1B? (note in Additional Submission Details field) - REQUIRED
• Weekend Requirements : call every 6th week, Monday to Monday (Phone call only)

• Open to experienced Board Certified provider or TRULY Board Eligible recent graduates
  
• If candidate is TRULY Board Eligible, must become Board Certified within 2 years of becoming eligible
